13         Senate Amendment 
14         On page 7, line 30,
15  
16  insert:  
17         (i)1.  Procedures to require accurate reporting to the
18  division of injuries incurred by greyhounds that race at any
19  facility that is licensed as a slot machine facility in this
20  state. The reports shall include:
21         a.  The greyhound's registered name and right and left
22  ear tattoo numbers;
23         b.  The name, business address, and telephone number of
24  the owner, trainer, and kennel operator of the greyhound;
25         c.  The color, weight, and sex of the greyhound;
26         d.  The specific type of injury, cause of the injury,
27  estimated recovery time, and location of the injury on the
28  greyhound;
29         e.  Where the injury occurred, whether on a racing
30  track or in another area;
31         f.  If the injury occurred while the greyhound was

Florida Senate - 2005 SENATOR AMENDMENT Bill No. SB 4-B Barcode 934352 1 racing, the racetrack where the injury occurred; the distance, 2 grade, race, and post position of the greyhound when the 3 injury occurred; and the weather conditions, time, 4 temperature, and track condition at the time of injury; and 5 g. A certification by the racetrack veterinarian that 6 the form is correct. 7 2. The division shall maintain injury records for a 8 period of 7 years. 9 3. In addition to other penalties imposed by law, a 10 person who knowingly makes a false statement on an injury 11 report commits a misdemeanor of the first degree, for a first 12 violation, punishable as provided in s. 775.082 or s. 775.083, 13 and a felony of the third degree, for a subsequent violation, 14 punishable as provided in s. 775.082, s. 775.083, or s. 15 775.084. 16 17 18 19 20 21 22 23 24 25 26 27 28 29 30 31 2 6:33 PM 12/07/05 s0004Bb-05-e0a
