A bill to be entitled
2An act relating to motor vehicles; amending s. 320.08056,
3F.S.; revising the annual use fee for the Choose Life
4license plate; establishing annual use fees for specified
5license plates; limiting the use of proceeds from the sale
6of specialty license plates; amending s. 320.08058, F.S.;
7revising authorized uses of proceeds received from the
8sale of the Choose Life license plate, United We Stand
9license plate, and Florida Golf license plate; creating an
10Autism license plate, a Let's Go Surfing license plate, a
11Florida Horse Park license plate, a St. Johns River
12license plate, an Endless Summer license plate, a Catch
13Me, Release Me license plate, a Fraternal Order of Police
14license plate, a Go Green Florida license plate, and a
15Florida Biodiversity Foundation license plate; providing
16for the distribution of use fees received from the sale of
17such plates; amending s. 320.02, F.S.; requiring the
18application form for motor vehicle registration and
19renewal of registration to include language permitting a
20voluntary contribution to the Autism License Plate Fund
21and Ronald McDonald Houses of Florida; providing for
22distribution of such contributions; amending s. 320.0706,
23F.S.; providing for the display of license plates on
24wreckers; providing penalties; amending s. 322.08, F.S.;
25requiring the application form for a driver's license or
26duplicate thereof to include language permitting a
27voluntary contribution to the Autism License Plate Fund;
28providing an effective date.

30Be It Enacted by the Legislature of the State of Florida:
31
32     Section 1.  Paragraph (cc) of subsection (4) of section
33320.08056, Florida Statutes, is amended, paragraphs (qqq)
34through (yyy) are added to that subsection, and subsections (13)
35and (14) are added to that section, to read:
36     320.08056  Specialty license plates.--
37     (4)  The following license plate annual use fees shall be
38collected for the appropriate specialty license plates:
39     (cc)  Choose Life license plate, $25 $20.
40     (qqq)  Autism license plate, $25.
41     (rrr)  Let's Go Surfing license plate, $25.
42     (sss)  Florida Horse Park license plate, $25.
43     (ttt)  St. Johns River license plate, $25.
44     (uuu)  Endless Summer license plate, $25.
45     (vvv)  Catch Me, Release Me license plate, $25.
46     (www)  Fraternal Order of Police license plate, $25.
47     (xxx)  Go Green Florida license plate, $25.
48     (yyy)  Florida Biodiversity Foundation license plate, $25.
49     (13)  Effective July 1, 2011, in addition to the
50requirements of subsections (10) and (11) and notwithstanding
51any specific authority provided in s. 320.08058, an organization
52or entity receiving annual use fee revenue from the sale of a
53specialty license plate may use no more than a total of 10
54percent of the revenues for administrative, marketing, and
55promotion costs, including annual audit and compliance affidavit
56costs.
57     (14)  Notwithstanding any other provision of this chapter,
58no annual use fee revenue derived from the sale of a specialty
59license plate, or the interest thereon, shall be expended on
60projects or programs outside this state.
61     Section 2.  Subsection (29), paragraph (b) of subsection
62(32), and subsection (35) of section 320.08058, Florida
63Statutes, are amended, and subsections (69) through (77) are
64added to that section, to read:
65     320.08058  Specialty license plates.--
66     (29)  CHOOSE LIFE LICENSE PLATES.--
67     (a)  The department shall develop a Choose Life license
68plate as provided in this section. The word "Florida" must
69appear at the bottom of the plate, and the words "Choose Life"
70must appear at the top of the plate.
71     (b)  The annual use fees shall be distributed quarterly to
72Choose Life, Inc., a s. 501(c)(3) nonprofit corporation, with a
73list of annually to each county in the ratio that the annual use
74fees collected by each county bears to the total fees collected
75for the plates within the state.
76     (c)  Up to 10 percent of the fees may be used for
77administrative costs directly associated with the operations of
78Choose Life, Inc., and activities by Choose Life, Inc., to
79promote adoption and to market the license plate in this state.
80Choose Life, Inc., shall administer distribution of the
81remaining Each county shall distribute the funds to
82nongovernmental, not-for-profit agencies within the county,
83which agencies' services primarily consist of are limited to
84counseling and meeting the physical needs of pregnant women who
85are considering committed to placing their children for
86adoption.
87     1.  Funds may not be distributed to any agency that is
88involved or associated with abortion activities, including
89counseling for or referrals to abortion clinics, providing
90medical abortion-related procedures, or proabortion advertising,
91and funds may not be distributed to any agency that charges
92women for services received.
93     2.  Choose Life, Inc., may, from time to time, establish
94criteria pertaining to appropriate percentages of expenditures
95on items including, but not limited to, physical needs of women,
96physical needs of infants awaiting adoption, counseling,
97advertising, and training.
98     3.  Choose Life license plate funds generated in counties
99that do not have a qualified service provider shall be
100distributed by Choose Life, Inc., to qualified service providers
101in other counties.
102     4.  Funds shall not be removed from a county with active
103qualified service providers for distribution to service
104providers in other counties unless year-end unused fund balances
105exceed 20 percent of the county's annual Choose Life license
106plate revenues for 3 consecutive years. If year-end fund
107balances exceed 20 percent of a county's annual revenues for 3
108consecutive years, such balance may be distributed by Choose
109Life, Inc., to qualified service providers in other counties.
110     1.  Agencies that receive the funds must use at least 70
111percent of the funds to provide for the material needs of
112pregnant women who are committed to placing their children for
113adoption, including clothing, housing, medical care, food,
114utilities, and transportation. Such funds may also be expended
115on infants awaiting placement with adoptive parents.
116     2.  The remaining funds may be used for adoption,
117counseling, training, or advertising, but may not be used for
118administrative expenses, legal expenses, or capital
119expenditures.
120     5.3.  Each service provider agency that receives such funds
121must submit an annual attestation to Choose Life, Inc., which
122shall, upon request, make copies available to a state agency
123charged with auditing the county. Any unused funds that exceed
12410 percent of the funds received by an agency during its fiscal
125year must be returned to the county, which shall distribute them
126to other qualified agencies.
127     6.  Choose Life license plate use fees generated in this
128state may only be spent on activities within this state.
129     (d)1.  Within 90 days after the effective date of this
130paragraph, county commissions that have never distributed Choose
131Life funds shall forward such funds to Choose Life, Inc., for
132distribution.
133     2.  Within 180 days after the effective date of this
134paragraph, county commissions that have been distributing Choose
135Life funds shall forward fund balances to Choose Life, Inc., for
136distribution.
137     (32)  UNITED WE STAND LICENSE PLATES.--
138     (b)  The department shall retain all revenues from the sale
139of such plates until all startup costs for developing and
140issuing the plates have been recovered. Thereafter,
141notwithstanding s. 332.14, 100 percent of the annual use fee
142shall be distributed to the Department of Transportation SAFE
143Council for the development and implementation of programs to
144fund a grant program to enhance security at airports throughout
145the state, pursuant to s. 332.14. Up to 10 percent of the annual
146use fee revenue may be used for administration, promotion, and
147marketing of the plate, including annual audit and compliance
148affidavit costs.
149     (35)  FLORIDA GOLF LICENSE PLATES.--
150     (a)  The Department of Highway Safety and Motor Vehicles
151shall develop a Florida Golf license plate as provided in this
152section. The word "Florida" must appear at the bottom of the
153plate. The Dade Amateur Golf Association, following consultation
154with the PGA TOUR, the Florida Sports Foundation, the LPGA, and
155the PGA of America may submit a revised sample plate for
156consideration by the department.
157     (b)  The department shall distribute the Florida Golf
158license plate annual use fee to the Dade Amateur Golf
159Association, a s. 501(c)(3) organization Florida Sports
160Foundation, a direct-support organization of the Office of
161Tourism, Trade, and Economic Development. The license plate
162annual use fees are to be annually allocated as follows:
163     1.  Up to 10 5 percent of the proceeds from the annual use
164fees may be used by the Dade Amateur Golf Association Florida
165Sports Foundation for the administration of the Florida Junior
166Youth Golf Program.
167     2.  The Dade Amateur Golf Association shall receive the
168first $80,000 in proceeds from the annual use fees for the
169operation of youth golf programs in Miami-Dade County.
170Thereafter, 15 percent of the proceeds from the annual use fees
171shall be provided to the Dade Amateur Golf Association for the
172operation of youth golf programs in Miami-Dade County.
173     3.  The remaining proceeds from the annual use fees shall
174be available for grants to nonprofit organizations to operate
175youth golf programs and for marketing the Florida Golf license
176plates. All grant recipients, including the Dade Amateur Golf
177Association, shall be required to provide to the Dade Amateur
178Golf Association Florida Sports Foundation an annual program and
179financial report regarding the use of grant funds. Such reports
180shall be made available to the public.
181     (c)  The Dade Amateur Golf Association shall Florida Sports
182Foundation may establish a Florida Junior Golf Council Youth
183Golf Program. The Florida Junior Golf Council Youth Golf Program
184shall assist organizations for the benefit of youth, introduce
185young people to golf, instruct young people in golf, teach the
186values of golf, and stress life skills, fair play, courtesy, and
187self-discipline.
188     (d)  The Dade Amateur Golf Association Florida Sports
189Foundation shall establish a seven-member Florida Junior Golf
190Council committee to offer advice regarding the distribution of
191the annual use fees for grants to nonprofit organizations. The
192council advisory committee shall consist of one member from a
193group serving youth, one member from a group serving disabled
194youth, and five members at large.
195     (69)  AUTISM LICENSE PLATES.--
196     (a)  The department shall develop an Autism license plate
197as provided in this section. Autism license plates must bear the
198colors and design approved by the department. The word "Florida"
199must appear at the top of the plate, and the words "Support
200Autism Programs" must appear at the bottom of the plate.
201     (b)  The proceeds from the license plate annual use fee
202shall be distributed to Achievement and Rehabilitation Centers,
203Inc., to fund service programs for autism and related
204disabilities throughout the state and to operate and establish
205programs to support individuals with autism and related
206disabilities through direct services, evaluation, training, and
207awareness. Achievement and Rehabilitation Centers, Inc., shall
208establish an Autism Services Grant Council that shall provide
209grants from available Autism license plate proceeds to nonprofit
210organizations for direct services and programs for individuals
211with autism and related disabilities and their families.
212Consideration for participation in such services and programs
213shall be given to applicants who are children or adults with
214autism and related disabilities and their families and shall
215include those who are on the Agency for Persons with
216Disabilities waiting lists for services. Achievement and
217Rehabilitation Centers, Inc., shall also establish an Autism
218License Plate Fund. Moneys in the fund shall be used by the
219grant council as provided in this paragraph. All funds received
220under this subsection must be used in this state.
221     (c)  Achievement and Rehabilitation Centers, Inc., may
222retain all proceeds from the annual use fee up to $85,000 until
223all documented startup costs for developing and establishing the
224plate have been recovered. Thereafter, the proceeds from the
225annual use fee shall be used as follows:
226     1.  Up to 10 percent of the proceeds from the annual use
227fee may be used for the administration of the Autism License
228Plate Fund, the Autism Services Grant Council, and related
229matters.
230     2.  Funds may be used as necessary for annual audit or
231compliance affidavit costs.
232     3.  Thirty-five percent of the proceeds from the annual use
233fee shall be used to establish and operate programs to support
234individuals with autism and related disabilities and their
235families through direct services, evaluation, training, and
236awareness in the state.
237     4.  The Center for Autism and Related Disabilities at the
238University of Miami shall receive 15 percent of the proceeds
239from the annual use fee for distribution, as determined
240appropriate by the director of that center, to the seven
241regional autism centers created under s. 1004.55. The regional
242centers shall use the proceeds to support the services they
243provide.
244     5.  The remaining proceeds from the annual use fee shall be
245available to the Autism Services Grant Council for grants to
246nonprofit organizations to operate direct services programs for
247individuals with autism and related disabilities and their
248families and for marketing the Autism license plate. All grant
249recipients, including Achievement and Rehabilitation Centers,
250Inc., and the Center for Autism and Related Disabilities, shall
251be required to provide to the Autism Services Grant Council an
252annual program and financial report regarding the use of grant
253funds. Such reports shall be made available to the public.
254     (d)  Notwithstanding any provision of law to the contrary,
255the first $10,000 in proceeds generated from voluntary
256contributions to the Autism License Plate Fund authorized in ss.
257320.02 and 322.08 shall be retained by the department to offset
258the department's implementation costs.
259     (70)  LET'S GO SURFING LICENSE PLATES.--
260     (a)  The department shall develop a Let's Go Surfing
261license plate as provided in this section. Let's Go Surfing
262license plates must bear the colors and design approved by the
263department. The word "Florida" must appear at the top of the
264plate, and the words "Let's Go Surfing" must appear at the
265bottom of the plate.
266     (b)  The license plate annual use fees shall be distributed
267to East Coast Surfing Hall of Fame and Museum, Inc., to fund
268activities, programs, and projects to promote Florida's surfing-
269related tourism, preserve Florida's surfing history and
270heritage, preserve Florida's surf and shores, and honor
271lifeguards who patrol Florida's surf and shores to protect
272beachgoers, surfers, and others. East Coast Surfing Hall of Fame
273and Museum, Inc., may retain all revenue from the annual use
274fees until all startup costs for developing and establishing the
275plate have been recovered. Thereafter, up to 10 percent of the
276annual use fee revenue may be used for administration,
277promotion, and marketing of the plate, including annual audit
278and compliance affidavit costs.
279     (71)  FLORIDA HORSE PARK LICENSE PLATES.--
280     (a)  The department shall develop a Florida Horse Park
281license plate as provided in this section. Florida Horse Park
282license plates must bear the colors and design approved by the
283department. The word "Florida" must appear at the top of the
284plate, and the words "Discover Florida's Horses" must appear at
285the bottom of the plate.
286     (b)  The annual use fees shall be distributed to the
287Florida Agriculture Center and Horse Park Authority created by
288s. 570.952, which shall retain all proceeds until all startup
289costs for developing and establishing the plate have been
290recovered. Thereafter, the proceeds shall be used as follows:
291     1.  A maximum of 10 percent of the proceeds from the annual
292use fees may be used for the administration, promotion, and
293marketing of the Florida Horse Park license plate.
294     2.  The remaining proceeds shall be used by the authority
295to promote the Florida Agriculture Center and Horse Park located
296in Marion County; to support continued development of the park,
297including the construction of additional educational facilities,
298barns, and other structures; to provide improvements to the
299existing infrastructure at the park; and to provide for
300operational expenses of the Florida Agriculture Center and Horse
301Park.
302     (72)  ST. JOHNS RIVER LICENSE PLATES.--
303     (a)  The department shall develop a St. Johns River license
304plate as provided in this section. St. Johns River license
305plates must bear the colors and design approved by the
306department. The word "Florida" must appear at the top of the
307plate, and the words "St. Johns River" must appear at the bottom
308of the plate.
309     (b)  The requirements of s. 320.08053 must be met prior to
310the issuance of the plate. Thereafter, the license plate annual
311use fees shall be distributed to the St. Johns River Alliance,
312Inc., a s. 501(c)(3) nonprofit organization, which shall
313administer the fees as follows:
314     1.  The St. Johns River Alliance, Inc., shall retain the
315first $60,000 of the annual use fees as direct reimbursement for
316administrative costs, startup costs, and costs incurred in the
317development and approval process. Thereafter, up to 10 percent
318of the annual use fee revenue may be used for administrative
319costs directly associated with education programs, conservation,
320research, and grant administration of the organization and for
321promotion and marketing of the specialty license plate.
322     2.  At least 30 percent of the fees shall be available for
323competitive grants for targeted community-based or county-based
324research or projects for which state funding is limited or not
325currently available. The remaining 50 percent shall be directed
326toward community outreach and access programs. The competitive
327grants shall be administered and approved by the board of
328directors of the St. Johns River Alliance, Inc. A grant advisory
329committee shall be composed of six members chosen by the St.
330Johns River Alliance board members.
331     3.  Any remaining funds shall be distributed with the
332approval of and accountability to the board of directors of the
333St. Johns River Alliance, Inc., and shall be used to support
334activities contributing to education, outreach, and springs
335conservation.
336     (73)  ENDLESS SUMMER LICENSE PLATES.--
337     (a)  The department shall develop an Endless Summer license
338plate as provided in this section. Endless Summer license plates
339must bear the colors and design approved by the department. The
340word "Florida" must appear at the top of the plate, and the
341words "Endless Summer" must appear at the bottom of the plate.
342     (b)  The license plate annual use fees shall be distributed
343to Surfing's Evolution & Preservation Corporation to fund its
344activities, programs, and projects aimed at preserving the sport
345of surfing. Surfing's Evolution & Preservation Corporation may
346retain all revenue from the annual use fees until all startup
347costs for developing and establishing the plate have been
348recovered. Thereafter, up to 10 percent of the annual use fee
349revenue may be used for promotion and marketing of the specialty
350license plate and administrative costs directly associated with
351the corporation's programs and the specialty license plate.
352Surfing's Evolution & Preservation Corporation shall use the
353remaining funds as follows:
354     1.  To fund the proposed Surfing's Evolution & Preservation
355Experience project.
356     2.  To provide funds for the provision of lifeguards or the
357building of artificial reefs.
358     3.  To provide funds to organizations that house the
359history and artifacts of surfing or promote the sport through
360exhibits, lectures, and events.
361     4.  To support programs and events of other organizations
362that support beaches and oceans and promote education on beach
363safety, coastal pollution, and beach ecology.
364     (74)  CATCH ME, RELEASE ME LICENSE PLATES.--
365     (a)  The department shall develop a Catch Me, Release Me
366license plate as provided in this section. Catch Me, Release Me
367license plates must bear the colors and design approved by the
368department. The word "Florida" must appear at the top of the
369plate, and the words "Catch Me, Release Me" must appear at the
370bottom of the plate.
371     (b)  The license plate annual use fees shall be distributed
372to the Guy Harvey Ocean Foundation, Inc., to fund marine-related
373scientific research, including research of free-ranging pelagic
374marine species that inhabit, use, or migrate through Florida
375waters; conservation initiatives; and education and public
376outreach programs targeting school-aged children in the state.
377The Guy Harvey Ocean Foundation, Inc., may retain all revenue up
378to $60,000 from the annual use fees until all startup costs for
379developing and establishing the plate have been recovered.
380Thereafter, up to 10 percent of the annual use fee revenue may
381be used for administrative costs directly associated with the
382operations of the Guy Harvey Ocean Foundation, Inc., and up to
38310 percent may be used for promotion and marketing of the
384specialty license plate.
385     (75)  FRATERNAL ORDER OF POLICE LICENSE PLATES.--
386     (a)  The department shall develop a Fraternal Order of
387Police license plate as provided in this section. Fraternal
388Order of Police license plates must bear the colors and design
389approved by the department. The word "Florida" must appear at
390the top of the plate, and the words "Fraternal Order of Police"
391must appear at the bottom of the plate.
392     (b)  In order to purchase the Fraternal Order of Police
393license plate, a person must submit a certified letter from the
394Florida State Lodge of the Fraternal Order of Police showing
395that he or she qualifies for the use of the Fraternal Order of
396Police law enforcement emblem by being a member in good
397standing.
398     (c)  The license plate annual use fees shall be distributed
399to the Florida State Lodge Fraternal Order of Police Memorial
400Foundation to fund its activities, programs, and projects aimed
401at preserving, maintaining, and expanding the Florida Fraternal
402Order of Police Law Enforcement Memorial. The Florida State
403Lodge of the Fraternal Order of Police may retain all revenue
404from the annual use fees until all startup costs for developing
405and establishing the plate have been recovered. Thereafter, up
406to 10 percent of the annual use fee revenue may be used for
407promotion and marketing of the specialty license plate and
408administrative costs directly associated with memorial programs
409and the specialty license plate. The Florida State Lodge  
410Fraternal Order of Police Memorial Foundation shall use the
411remaining funds as follows:
412     1.  To fund the Florida State Fraternal Order of Police Law
413Enforcement Memorial projects.
414     2.  To provide funds for the expansion, maintenance, and
415support of the Florida State Lodge Fraternal Order of Police
416Memorial Foundation.
417     3.  To provide funds to organizations that help support the
418families of law enforcement officers who have died in the line
419of duty.
420     4.  To support programs and events that support the Florida
421Fraternal Order of Police Law Enforcement Memorial.
422     (76)  GO GREEN FLORIDA LICENSE PLATES.--
423     (a)  The department shall develop a Go Green Florida
424license plate as provided in this section. Go Green Florida
425license plates must bear the colors and design approved by the
426department. The word "Florida" must appear at the top of the
427plate, the words "Go Green" must appear on the bottom of the
428plate, and the "Go Green Florida" emblem or logo must appear on
429the plate.
430     (b)  The requirements of s. 320.08053 must be met prior to
431the issuance of the plate. The annual use fees shall be
432distributed to the Coalition for Renewable Energy Solutions,
433Inc. After reimbursement for documented costs expended to
434establish the plate, the proceeds of the annual use fees shall
435be used in the following manner:
436     1.  Up to 10 percent may be used by the Coalition for
437Renewable Energy Solutions, Inc., for administrative, promotion,
438and marketing costs.
439     2.  The remainder shall be used to fund the Renewable
440Energy and Energy-Efficient Technologies Grants Program pursuant
441to s. 377.804, the Solar Energy System Incentives Program
442pursuant to s. 377.806, under the Florida Energy and Climate
443Commission, or other educational programs dedicated to providing
444effective alternative energy practices.
445     (c)  The coalition shall comply with the financial audit
446requirements of s. 320.08062.
447     (d)  The "Go Green Florida" emblem or logo may be
448authorized for use by the department in recognition of energy-
449efficient vehicle usage and practices. The display of the decal
450for energy-efficient practices may not be considered a
451substitute for a legal license tag.
452     (77)  FLORIDA BIODIVERSITY FOUNDATION LICENSE PLATES.--
453     (a)  The department shall develop a Florida Biodiversity
454Foundation license plate as provided in this section. Florida
455Biodiversity Foundation license plates must bear the colors and
456design approved by the department. The word "Florida" must
457appear at the top of the plate, and the words "Save Wild
458Florida" must appear at the bottom of the plate.
459     (b)  The annual use fees shall be distributed to the
460Florida Biodiversity Foundation, Inc., which shall retain 50
461percent of the proceeds until the startup costs to develop and
462establish the plates have been recovered. Thereafter, the
463proceeds shall be used as follows:
464     1.  A maximum of 10 percent of the proceeds may be used to fund
465fund the administrative, promotion, and marketing costs of the
466license plate program and the foundation.
467     2.  The remaining fees shall be used by the foundation to fund
468fund research, education, and scientific study of the diversity
469of animals and plants and to aid in the preservation, study,
470conservation, and recovery of imperiled organisms.
471     Section 3.  Paragraphs (h) and (i) are added to subsection
472(15) of section 320.02, Florida Statutes, to read:
473     320.02  Registration required; application for
474registration; forms.--
475     (15)
476     (h)  The application form for motor vehicle registration
477and renewal of registration must include language permitting a
478voluntary contribution of $5 to the Autism License Plate Fund
479administered by Achievement and Rehabilitation Centers, Inc.
480Except as provided in s. 320.08058(69)(d), such contributions
481must be distributed by the department each month to the Autism
482License Plate Fund administered by Achievement and
483Rehabilitation Centers, Inc., to be used as provided in s.
484320.08058(69).
485     (i)  The application form for motor vehicle registration
486and renewal of registration must include language permitting a
487voluntary contribution of $1 to Ronald McDonald Houses of
488Florida. Such contributions must be distributed by the
489department each month to Ronald McDonald House Charities Tampa
490Bay, Inc., a not-for-profit organization.
491
492For the purpose of applying the service charge provided in s.
493215.20, contributions received under this subsection are not
494income of a revenue nature.
495     Section 4.  Section 320.0706, Florida Statutes, is amended
496to read:
497     320.0706  Display of license plates on trucks.--The owner
498of any commercial truck of gross vehicle weight of 26,001 pounds
499or more shall display the registration license plate on both the
500front and rear of the truck in conformance with all the
501requirements of s. 316.605 that do not conflict with this
502section. The owner of a dump truck may place the rear license
503plate on the gate no higher than 60 inches to allow for better
504visibility. However, the owner of a truck tractor shall be
505required to display the registration license plate only on the
506front of such vehicle. The owner of any wrecker issued a regular
507wrecker license plate shall display the license plate only on
508the front of the vehicle. The owner of any wrecker issued gross
509vehicle weight wrecker license plates shall display the license
510plates on both the front and rear of the vehicle in conformance
511with the requirements of s. 316.605 that do not conflict with
512this section. A violation of this section is a noncriminal
513traffic infraction, punishable as a moving violation as provided
514in chapter 318.
515     Section 5.  Subsection (6) of section 322.08, Florida
516Statutes, is amended to read:
517     322.08  Application for license.--
518     (6)  The application form for a driver's license or
519duplicate thereof shall include language permitting the
520following:
521     (a)  A voluntary contribution of $1 per applicant, which
522contribution shall be deposited into the Health Care Trust Fund
523for organ and tissue donor education and for maintaining the
524organ and tissue donor registry.
525     (b)  A voluntary contribution of $1 per applicant, which
526contribution shall be distributed to the Florida Council of the
527Blind.
528     (c)  A voluntary contribution of $2 per applicant, which
529shall be distributed to the Hearing Research Institute,
530Incorporated.
531     (d)  A voluntary contribution of $1 per applicant, which
532shall be distributed to the Juvenile Diabetes Foundation
533International.
534     (e)  A voluntary contribution of $1 per applicant, which
535shall be distributed to the Children's Hearing Help Fund.
536     (f)  A voluntary contribution of $1 per applicant, which
537shall be distributed to Family First, a nonprofit organization.
538     (g)  A voluntary contribution of $5 per applicant, which,
539except as provided in s. 320.08058(69)(d), shall be distributed
540to the Autism License Plate Fund administered by Achievement and
541Rehabilitation Centers, Inc., to be used as provided in s.
542320.08058(69).

544A statement providing an explanation of the purpose of the trust
545funds shall also be included. For the purpose of applying the
546service charge provided in s. 215.20, contributions received
547under paragraphs (b), (c), (d), (e), and (f), and (g) and under
548s. 322.18(9)(a) are not income of a revenue nature.
549     Section 6.  This act shall take effect September 1, 2009.
