       The Committee on Budget Subcommittee on Health and Human
       Services Appropriations (Gaetz) recommended the following:
       
    1         Senate Amendment 
    2  
    3         Delete lines 1453 - 1484
    4  and insert:
    5         (3) In addition to the formulary of topical ocular
    6  pharmaceutical agents in subsection (2), a statutory formulary
    7  of oral pharmaceutical agents is established, which includes the
    8  following agents:
    9         (a) The following analgesics, or their generic or
   10  therapeutic equivalents, which may not be administered or
   11  prescribed for more than 72 hours:
   12         1. Tramadol hydrochloride.
   13         2. Acetaminophen 300 mg with No. 3 codeine phosphate 30 mg.
   14  
   15  If any of the oral analgesic agents set forth in this paragraph
   16  are administered or prescribed, the certified optometrists must
   17  immediately notify the patient’s primary care physician or a
   18  physician licensed under chapter 458 or chapter 459 who is
   19  skilled in diseases of the eye.
   20         (b) The following antibiotics, or their generic or
   21  therapeutic equivalents:
   22         1. Amoxicillin.
   23         2. Azithromycin.
   24         3. Ciproflaxacin.
   25         4. Dicloxacillin.
   26         5. Doxycycline.
   27         6. Keflex.
   28         7. Minocycline.
   29         (c) The following antivirals, or their generic or
   30  therapeutic equivalents:
   31         1. Acyclovir.
   32         2. Famciclovir.
   33         3. Valacyclovir.
   34         (d) The following oral anti-glaucoma agents, or their
   35  generic or therapeutic equivalents, which may not be
   36  administered or prescribed for more than 72 hours:
   37         1. Acetazolamide.
   38         2. Methazolamide.

   40  If any of the oral anti-glaucoma agents set forth in this
   41  paragraph are administered or prescribed, the certified
   42  optometrist must immediately notify the patient’s primary care
   43  physician or a physician licensed under chapter 458 or chapter
   44  459 who is skilled in diseases of the eye.
