       The Committee on Budget Subcommittee on Health and Human
       Services Appropriations (Garcia) recommended the following:
       
    1         Senate Amendment (with title amendment)
    2  
    3         Between lines 673 and 674
    4  insert:
    5  Section 17. Subsection (15) is added to section 395.4025,
    6  Florida Statutes, to read:
    7         395.4025 Trauma centers; selection; quality assurance;
    8  records.—
    9         (15) A public teaching hospital that operates facilities on
   10  separate premises under a single license and that has a level 1
   11  trauma center on one of the premises is exempt from the
   12  requirements of subsection (2) and a separate application is not
   13  required for the initiation of trauma services at another
   14  facility included on the single license, subject to the
   15  following:
   16         (a) The hospital must certify to the agency that it will
   17  meet and continuously maintain the critical elements required
   18  for a trauma center, including, but not limited to:
   19         1. The equipment and physical facilities necessary to
   20  provide trauma services;
   21         2. The personnel in sufficient numbers and having proper
   22  qualifications to provide trauma services; and
   23         3. An effective quality assurance process.
   24         (b) The hospital must provide documentation to the agency
   25  of the manner in which it will extend its existing trauma
   26  services to the additional hospital facility listed on the
   27  single hospital license.
   28         (c) The hospital must provide further documentation to the
   29  agency that demonstrates there were at least 350 trauma cases
   30  within a 5-mile radius of the location of the facility for which
   31  the exemption is claimed during the most recent 12-month period
   32  for which data is available, by the zip code of:
   33         1. The patient’s residence as reported by the agency
   34  hospital patient database; or
   35         2. Where the incident occurred, as reported by the
   36  emergency medical services provider.

   46         from the requirement for a separate application for
   47         recognition as a trauma center by the Agency for
   48         Health Care Administration; amending s.
