Health Care Coverage; Providing reimbursement rates applicable to payments by insurers for covered health care services provided in a hospital by physicians who are not members of a preferred provider network or exclusive provider network; providing reimbursement rates applicable to payments by health maintenance organizations for covered health care services provided in a hospital setting by physicians who do not have a contract with the health maintenance organization; providing requirements and limitations with respect to the collection of fees or payments for such services, etc.
