Dentists; Prohibits contract between health insurer, prepaid limited health service organization, or HMO & dentist from requiring dentist to provide services at set fee unless the services are covered services; provides covered services are those services listed as entitled benefit under contract; prohibits insurer from providing merely de minimis reimbursement or coverage; requires that fees for covered services be set in good faith & not be nominal; prohibits health insurer, prepaid limited health service organization, or HMO from requiring as condition of contract that dentist participate in discount medical plan; provides for applicability.
