14         On page 13, line 11, after the period

16  insert:

17  However, such permanent facilities must file with the

18  department by March 1 of each year a written report, on a form

19  prescribed by rule of the department, of any mechanical,

20  structural, or electrical defects affecting patron safety for

21  which an amusement ride was closed for patron use for a period

22  of time in excess of 4 hours which occurred during the

23  previous calendar year, and any accident, of which the owner

24  or manager had knowledge, or through the exercise of

25  reasonable diligence should have had knowledge, in which a

26  patron was transported to a hospital, as defined in chapter

27  395, which occurred during the previous calendar year.
