14         On page 3, lines 15-20, delete those lines

16  and insert:

17         (a)  Notification of services. This element provides

18  for:

19         1.  Notification of services prenatally or at birth.

20         2.  Notification of available services through

21  distribution of an informational brochure on the program.  No

22  assessment tool shall be utilized and the voluntary nature of

23  participation in available services must be clearly stated on

24  the informational brochure and on any subsequent application

25  or paperwork completed by a participant.
