15         On page 2, lines 18-23, delete those lines

17  and insert:

18         (n)  Present the following disclaimer verbally and in

19  writing at the initial contact with the parent, which may

20  occur before or after the birth of a child:  "Participation in

21  the initial interview and assessment process and all services

22  provided through the Healthy Families Florida program is

23  voluntary. You have the right not to answer any questions

24  asked during the assessment process and the right to decline

25  to participate in the program at any time."
