14 On page 3, lines 11-22, delete those lines

16 and insert: health outcomes. The following disclaimer shall
17 be presented verbally and in writing at the initial contact
18 with the parent, which may occur before or after the birth of
19 a child: "Participation in the initial interview and
20 assessment process and all services provided through the
21 Healthy Families Florida program is voluntary. You have the
22 right not to answer any questions asked during the assessment
23 process and the right to decline to participate in the program
24 at any time."
25 (4) PROGRAM REQUIREMENTS.--The program shall provide
26 for intensive home visits and include the following critical
27 elements:
28 (a) Initiation of services. This element provides for:
29 1. Initiation of services prenatally or at birth.
30 2. Use of a standardized assessment tool to
31 systematically identify those families most in need of
