11  Senator Williams moved the following amendment:

13         Senate Amendment 

14         On page 9, lines 21-25, delete those lines

15

16  and insert:

17         (l)  Evaluate the progress of the program and provide

18  an annual report regarding the progress and achievement of

19  designated outcomes, based upon the evaluation design set

20  forth in paragraph (i), to the Governor, the President of the

21  Senate, the Speaker of the House of Representatives, the

22  Office of Program Policy Analysis and Government

23  Accountability, and other vested parties.

24         (m)  Include the following outcome measures for each

25  service delivery area in the evaluation design required by

26  paragraph (i):

27         1.  The percentage of children in families

28  participating in the Healthy Families Florida program who are

29  not abused or neglected while receiving services within 6, 12,

30  and 18 months of program completion.

31         2.  The percentage reduction in the total number of

 1  confirmed reports of abuse and neglect by the Department of

 2  Children and Family Services.

 3         3.  The percentage of children receiving immunizations

 4  and receiving well-child care.

 5         4.  The percentage of participants removed from public

 6  assistance programs.

 7         5.  The percentage of participants who are added to

 8  public assistance programs.

 9         6.  The percentage of teen mother participants with no

10  subsequent pregnancy.

11         7.  The average amount of time participants are in the

12  program.

13         8.  The percentage of participants satisfied with the

14  program.

16  Additional outcome measures may be recommended by the Healthy

17  Families Florida Advisory Committee.

18         (n)  Present the following disclaimer both verbally and

19  in writing at the initial contact with the parent:

20  "PARTICIPATION IN THE HEALTHY FAMILIES FLORIDA PROGRAM IS

21  VOLUNTARY.  YOU ARE NOT REQUIRED TO ANSWER ANY QUESTIONS OTHER

22  THAN THOSE REQUIRED FOR BIRTH REGISTRATION AND YOU HAVE THE

23  RIGHT TO DECLINE PARTICIPATION IN THE PROGRAM AT ANY TIME."

24         (o)  Furnish, at the participant's request, a copy of

25  all documentation concerning services provided to the

26  participant, including applications and assessments.
