11  Senator Campbell moved the following amendment:

13         Senate Amendment 

14         On page 7, lines 15-17, delete those lines

15

16  and insert:  to s. 743.015, or similar statutes of other

17  states;

18         4.  Notice is waived if the minor declares in a written

19  or oral statement that she is a victim of child abuse or

20  neglect, or sexual abuse by one or both of her parents, her

21  guardian, or her custodian, or of neglect, abuse or domestic

22  violence under s. 741.28 against herself or any of her family

23  members by another family or household member as defined in s.

24  741.28. The doctor must record the statement in the patient's

25  medical record. Any physician relying in good faith on a

26  written or oral statement under this subsection shall not be

27  liable for disciplinary action under this subsection for

28  failure to give notice; or

29         5.  Notice is waived under the provisions of subsection

30  (5).
