11  Senator Campbell moved the following amendment:

13         Senate Amendment 

14         On page 7, lines 20-22, delete those lines

15

16  and insert:  dependent on her;

17         5.  Notice is waived if the minor declares in a written

18  or oral statement that she is a victim of child abuse or

19  neglect, or sexual abuse by one or both of her parents, her

20  guardian, or her custodian, or of neglect, abuse or domestic

21  violence under s. 741.28 against herself or any of her family

22  members by another family or household member as defined in s.

23  741.28. The doctor must record the statement in the patient's

24  medical record. Any physician relying in good faith on a

25  written or oral statement under this subsection shall not be

26  liable for disciplinary action under this subsection for

27  failure to give notice; or

28         6.  Notice is waived under the provisions of subsection

29  (5).
