11  Senator Campbell moved the following amendment:

13         Senate Amendment (with title amendment) 

14         On page 7, line 3, delete that line

15

16  and insert:

17         (b)  The referring physician may not provide the

18  notification required under this section until at least 72

19  hours after the physician has informed the minor of the

20  notification requirements of this section and given the minor

21  a written statement of the circumstances under which

22  notification may be waived including the process necessary for

23  judicial waiver of notice under subsection (5). The written

24  statement must include the telephone numbers of the Florida

25  Bar Attorney Referral Service and the Court Administrator's

26  Office for the county in which the minor resides. The

27  requirements of this subsection may be waived by the minor

28  signing a written statement that she has no objection to the

29  physician complying immediately with the notification

30  requirements of this section.

31         (c)  Notice shall not be required if:
