15         On page 13, line 13, after the period

17  insert:

18  However, such permanent facilities must file with the

19  department by March 1 of each year a written report, on a form

20  prescribed by rule of the department, of any mechanical,

21  structural, or electrical defects affecting patron safety for

22  which an amusement ride was closed for patron use for a period

23  of time in excess of 4 hours which occurred during the

24  previous calendar year, and any accident, of which the owner

25  or manager had knowledge, or through the exercise of

26  reasonable diligence should have had knowledge, in which a

27  patron was transported to a hospital, as defined in chapter

28  395, which occurred during the previous calendar year.
