1 A bill to be entitled
2 An act relating to health care coverage;
3 requiring certain health insurance policies,
4 health care services plans, or other contracts
5 to provide coverage for scalp hair prostheses;
6 providing an effective date.

8 Be It Enacted by the Legislature of the State of Florida:

10 Section 1. (1) Any health insurance policy, health
11 care services plan, or other health care contract issued or
12 renewed in this state which provides benefits for medical or
13 hospital expenses and which also provides coverage for
14 prostheses must provide coverage for expenses for scalp hair
15 prostheses worn for hair loss suffered as a result of alopecia
16 areata, alopecia totalis, or permanent loss of scalp hair due
17 to injury or illness. Such coverage, however, is subject to a
18 written recommendation by the treating physician that the
19 prosthesis is a medical necessity, and coverage is subject to
20 the same limitations and guidelines as other prostheses.
21 (2) As used in this section, the term:
22 (a) "Prosthesis" means an artificial appliance used to
23 replace lost natural structures, including artificial arms,
24 legs, breasts or glass eyes.
25 (b) "Scalp hair prosthesis" means an artificial
26 substitute for scalp hair that is made for a specific
27 individual.
28 Section 2. This act shall take effect July 1, 1998.
