16 and insert:
17 (4) The consent for voluntary surrender must be
18 executed only after the birth of the minor, child, in the
19 presence of two witnesses, and must be acknowledged before a
20 notary public. The consent for voluntary surrender becomes
21 binding 48 hours after execution. The notary public must
22 legibly note on the consent the date and time the consent was
23 executed. The witnesses' names must be typed or printed
24 underneath their signatures, and their home or business
25 addresses, and social security numbers, and drivers' license
26 numbers must be included. The consent must
