11  The Committee on Health, Aging and Long-Term Care recommended

12  the following amendment:

14         Senate Amendment 

15         On page 4, lines 9-14; page 5, lines 23-28; page 7,

16  lines 7-11; page 8, lines 20-25 delete:  If an applicant has

17  submitted fingerprints to the department for a national

18  criminal history check upon initial licensure and is renewing

19  his or her license for the first time, then the applicant need

20  only submit the information and fee required for a statewide

21  criminal history check.

23  and insert:

24         If an applicant has received an initial license to

25  practice in this state after January 1, 1992 and has submitted

26  fingerprints to the department for a national criminal history

27  check upon initial licensure and is renewing his or her

28  license for the first time, then the applicant need only

29  submit the information and fee required for a statewide

30  criminal history check.
