17 Section 72. Subsection (1) of section 322.051, Florida
18 Statutes, is amended to read:
19 322.051 Identification cards.--
20 (1) Any person who is 12 years of age or older, or any
21 person who has a disability, regardless of age, who applies
22 for a disabled parking permit under s. 320.0848, may be issued
23 an identification card by the department upon completion of an
24 application and payment of an application fee.
25 (a) Each such application shall include the following
26 information regarding the applicant:
27 1. Full name (first, middle or maiden, and last),
28 gender, social security card number, residence and mailing
29 address, and a brief description.
30 2. Proof of birth date satisfactory to the department.
31 3. Proof of identity satisfactory to the department,

1 including one of the following: a certified copy of a United
2 States birth certificate, a valid United States passport, an
3 alien registration receipt card (green card), an employment
4 authorization card issued by the United States Department of
5 Justice, or proof of nonimmigrant classification provided by
6 the United States Department of Justice, for an original
7 identification card. The application must include the
8 applicant's full name (first, middle or maiden, and last),
9 sex, race, residence address and mailing address, proof of
10 birth satisfactory to the department, and other data that the
11 department requires.
12 (b) An application for an identification card must be
13 signed and verified by the applicant in a format designated by
14 the department before a person authorized to administer oaths.
15 The fee for an identification card is $3, including payment
16 for the color photograph or digital image of the applicant.
17 Section 73. Subsection (2) of section 322.08, Florida
18 Statutes, 1998 Supplement, is amended to read:
19 322.08 Application for license.--
20 (2) Each such application shall include the following
21 information regarding the applicant:
22 (a) Full name (first, middle or maiden, and last),
23 gender, social security card number, residence and mailing
24 address, and a brief description.
25 (b) Proof of birth date satisfactory to the
26 department.
27 (c) Proof of identity satisfactory to the department,
28 including one of the following: a certified copy of a United

1 Justice, or proof of nonimmigrant classification provided by
2 the United States Department of Justice, for an original
3 license.
4 (d) Whether the applicant has previously been licensed
5 to drive, and, if so, when and by what state, and whether any
6 such license or driving privilege has ever been disqualified,
7 revoked, or suspended, or whether an application has ever been
8 refused, and, if so, the date of and reason for such
9 disqualification, suspension, revocation, or refusal. Each
10 such application shall reflect the full name (first, middle or
11 maiden, and last), proof of identity satisfactory to the
12 department, proof of birth date satisfactory to the
13 department, sex, social security number, and residence and
14 mailing address of the applicant, and briefly describe the
15 applicant, and shall state whether the applicant has
16 previously been licensed to drive, and if so, when and by what
17 state, and whether any such license or driving privilege has
18 ever been disqualified, revoked, or suspended, or whether an
19 application has ever been refused, and if so, the date of and
20 reason for such disqualification, suspension, revocation, or
21 refusal.
22 Section 74. Paragraph (b) of subsection (1) of section
23 322.09, Florida Statutes, is amended to read:
24 322.09 Application of minors.--
25 (1)
26 (b) There shall be submitted with each application a
27 certified copy of a United States birth certificate, a valid
28 United States passport, an alien registration receipt card
29 (green card), an employment authorization card issued by the
30 United States Department of Justice, or proof of nonimmigrant
31 classification provided by the United States Department of

1 Justice, for an original license the birth certificate of the
2 applicant. If the applicant is unable to furnish such
3 certified copy, a certificate from the public school
4 authorities as to the age of the applicant upon entering
5 school as required by s. 232.03, or the school authorities of
6 the state where applicant enrolled in school, shall be
7 submitted. Upon inability of applicant to establish a birth
8 date as above provided, then the same may be established in
9 the order of preference as provided by s. 232.03. However,
10 uncertified copies of such documents shall not be accepted.
