13         Amendment to Amendment (515085) 

14         On page 154, lines 19-25, and

15         on page 155, lines 10-15,

16  remove from the amendment:  All payments to a health care

17  provider by a fiscal intermediary must include an explanation

18  of services being reimbursed which includes, at a minimum, the

19  patient's name, the date of service, the provider code, the

20  amount of reimbursement for noncapitated providers, and the

21  identification of the plan on whose behalf the payment is

22  being made.

24  and insert in lieu thereof:  All payments to a health care
