14         On page 155, lines 12-18, and

15         on page 156, lines 3-9,

16  remove from the bill:  All payments to a health care provider

17  by a fiscal intermediary must include an explanation of

18  services being reimbursed which includes, at a minimum, the

19  patient's name, the date of service, the provider code, the

20  amount of reimbursement for noncapitated providers, and the

21  identification of the plan on whose behalf the payment is

22  being made.

24  and insert in lieu thereof:  All payments to a health care

25  provider by a fiscal intermediary for noncapitated providers

26  must include an explanation of services being reimbursed which

28  service, the procedure code, the amount of reimbursement, and
