15 remove from the bill: all of said lines

17 and insert in lieu thereof: grievance procedures, not to
18 exceed 30 days from the date of receipt of the notice of
19 grievance, before filing a petition for benefits under this
20 section. Failure to exhaust managed care grievance procedures,
21 not to exceed 30 days from the date of receipt of the notice
22 of grievance, shall result in dismissal of the petition
23 without prejudice.
24 (4) The petition must include a certification by the
25 claimant or, if the claimant is represented by counsel, the
26 claimant's attorney, stating that the claimant, or attorney if
27 the claimant is represented by counsel, has made a good faith
28 effort to resolve the dispute and that the claimant or
29 attorney was unable to resolve the dispute with the carrier.
30 The petition shall also include a certification by the
31 claimant or, if the claimant is represented by counsel, the
