11  Senator Webster moved the following amendment:

13         Senate Amendment (with title amendment) 

14         On page 11, line 31,

16  insert:

17         Section 12.  (1)  In order to implement Specific

18  Appropriation 363B of the 1999-2000 General Appropriations

19  Act, all Healthy Families Florida contracted service providers

20  shall:

21         (a)  Present the following disclaimer both orally and

22  in writing at the initial contact with the

23  parent:  "Participation in the Healthy Families Program is

24  voluntary. You are not required to answer any questions other

25  than those required for birth registration and you have the

26  right to decline participation in the program at any time."

27         (b)  Furnish, at the participant's request, a copy of

28  all documentation concerning services provided to the

29  participant, including applications and assessments. The

30  private, nonprofit corporation and other applicable service

31  providers shall dispose of all records or documents relating

 1  to that individual 5 years after termination from the program.

 2         (2)  No information other than the name, date of birth,

 3  social security number, zip code, and county of residence of

 4  participants and their children may be forwarded from the

 5  private, nonprofit corporation or other service provider to

 6  the Department of Children and Family Services. This

 7  information is to be used for evaluation purposes only. No

 8  individual participant data may be forwarded to the National

 9  Committee to Prevent Child Abuse or any other organization

10  collecting and recording such information.

11         (3)  This section expires July 1, 2000.
