11  Senator Webster moved the following amendment:

13         Senate Amendment (with title amendment) 

14         On page 11, line 31,

16  insert:

17         Section 12.  (1)  In order to implement Specific

18  Appropriation 363B of the 1999-2000 General Appropriations

19  Act, all Healthy Families Florida contracted service providers

20  shall:

21         (a)  Present the following disclaimer both orally and

22  in writing at the initial contact with the parent:

23         Participation in the Healthy Families Florida

24         Program is voluntary. You need not answer any

25         question other than those required for birth

26         registration, and you have the right to decline

27         participation in the program at any time.

28         (b)  Furnish, at the participant's request, a copy of

29  all documentation concerning services provided to the

30  participant, including applications and assessments. Upon

31  termination of an individual's participation in the Healthy

 1  Families Florida Program, the private, nonprofit corporation

 2  and other applicable service providers shall dispose of all

 3  records or documents relating to that individual.

 4         (2)  No information other than the name and age of a

 5  participant may be forwarded from the private, nonprofit

 6  corporation or other service provider to the Department of

 7  Children and Family Services, the National Committee to

 8  Prevent Child Abuse, or any other committee organization

 9  collecting and recording such information.

10         (3)  This section expires July 1, 2000.
