14         Senate Amendment 

15         On page 34, line 22, and

16         on page 54, line 11, after the period

17

18  insert:  The address of the parent's or guardian's local

19  school board and State Department of Education shall be

20  included on the report card. The report shall include a

21  statement established by the Commissioner of Education which

22  serves to encourage the student's parent or guardian to make

23  recommendations or provide comments to the Department of

24  Education and their local school board regarding the report

25  card and the plight of their child's school. The Department of

26  Education and the school district shall review parent feedback

27  and shall annually produce a report to the Legislature

28  concerning the parent feedback.
