13         Amendment to Amendment 565259 

14         On page 2, lines 22-25,

15  remove from the amendment:  all of said lines,

16

17  and insert in lieu thereof:  massage is prescribed by a

18  contracted physician licensed under chapter 458, chapter 459,

19  chapter 460, or chapter 461 as medically necessary and the

20  prescription specifies the number of treatments. Such massage

21  services shall be subject to the same terms, conditions, and

22  limitations as other covered services.
