12 the following amendment:

15 On page 145, between lines 12 and 13,

17 insert:
18 Section 99. Subsection (36) is added to section
19 641.31, Florida Statutes, 1998 Supplement, to read:
20 641.31 Health Maintenance contracts.--
21 (36) All health maintenance contracts that provide
22 coverage for massage shall also cover the services of persons
23 licensed to practice massage pursuant to chapter 480, if the
24 massage is prescribed by a physician licensed under chapter
25 458, chapter 459, chapter 460, or chapter 461 as medically
26 necessary and the prescription specifies the number of
27 treatments. Such massage services shall be subject to the same
28 terms, conditions, and limitations as other contracted
29 providers.
