11  The Committee on Health Care Services offered the following:

13         Amendment 

14         On page 4, lines 10-23,

15  remove from the bill:  all of said lines

16

17  and insert in lieu thereof:  organize local public and private

18  resources, and faith based organizations to address these

19  disparities, and evaluate the effectiveness of interventions.

20         (2)  It is therefore the intent of the Legislature to

21  provide funds within Florida counties and Front Porch Florida

22  Communities, in the form of Reducing Racial and Ethnic Health

23  Disparities: Closing the Gap grants, to stimulate the

24  development of community-based and neighborhood-based projects

25  which will improve the health outcomes of racial and ethnic

26  populations.  Further, it is the intent of the Legislature

27  that these programs foster the development of coordinated,

28  collaborative, and broad-based participation by public and

29  private entities, and faith based organizations.  Finally, it

30  is the intent of the Legislature that the grant program

31  function as a partnership between state and local governments,
