11  The Committee on Health Care Services offered the following:

13         Amendment 

14         On page 9, lines 6-22,

15  remove from the bill:  all of said lines

16

17  and insert in lieu thereof:

18         (1)  One or more Closing the Gap grants may be awarded

19  in a county, or in a group of adjoining counties from which a

20  multicounty application is submitted. Front Porch Florida

21  Communities grants may also be awarded in a county or group of

22  adjoining counties that is also receiving a grant award.

23         (2)  Closing the Gap grants shall be awarded on a

24  matching basis.  One dollar in local matching funds must be

25  provided for each $3 grant payment made by the state, except

26  that:

27         (a)  In counties with populations greater than 50,000,

28  up to 50 percent of the local match may be in-kind in the form

29  of free services or human resources. Fifty percent of the

30  local match must be in the form of cash.

31         (b)  In counties with populations of 50,000 or less,
