11  The Committee on Health Care Services offered the following:

13         Amendment 

14         On page 6, lines 5-24,

15  remove from the bill:  all of said lines

16

17  and insert in lieu thereof:  county may apply for a Closing

18  the Gap grant and may serve as the lead agency to administer

19  and coordinate project activities within the county and

20  develop community partnerships necessary to implement the

21  grant.

22         (2)  Persons, entities, or organizations within

23  adjoining counties with populations of less than 100,000,

24  based on the annual estimates produced by the Population

25  Program of the University of Florida Bureau of Economic and

26  Business Research, may jointly submit a multicounty Closing

27  the Gap grant proposal.  However, the proposal must clearly

28  identify a single lead agency with respect to program

29  accountability and administration.

30         (3)  In addition to the grants awarded under

31  subsections (1) and (2), up to 20 percent of the funding for

 2  Gap grant program shall be dedicated to projects that address

 3  improving racial and ethnic health status within specific

 4  Front Porch Florida Communities, as designated pursuant to s.

 5  14.2015(9)(b).
