11  Senator Forman moved the following amendment:

13         Senate Amendment (with title amendment) 

14         On page 4, before line 1,

15

16  insert:

17         Section 8.  (1)  The Department of Health, in

18  consultation with the Agency for Health Care Administration,

19  shall conduct an annual study on the termination of

20  pregnancies. The study shall collect annual information broken

21  down by county on the following:

22         (a)  The number of abortions performed in Florida;

23         (b)  The reason for the same;

24         (c)  The types of abortion procedures performed in

25  Florida;

26         (d)  The number of each type of abortion procedure

27  performed in Florida; and

28         (e)  The period of gestation at the time the procedures

29  were performed.

30         (2)  No personal information regarding the women who

31  terminate their pregnancies in Florida shall be sought by the
