15         On page 5, line 5, after the period

17  insert:  This account agreement shall include, but not be

18  limited to, the matching funds to be contributed to the

19  account, limits on the deposits for which the match will be

20  provided, required documentation necessary for payment of

21  moneys in the account to be made for a qualified purpose, and

22  penalties for withdrawal of funds not used for one or more of

23  the qualified purposes.
