16 and insert:
17 2. Beginning July 1, 2000, and until July 31, 2001,
18 offer and issue basic and standard small employer health
19 benefit plans on a guaranteed-issue basis to every eligible
20 small employer which is eligible for guaranteed renewal, has
21 less than two eligible employees, is not formed primarily for
22 the purpose of buying health insurance, elects to be covered
23 under such plan, agrees to make the required premium payments,
24 and satisfies the other provisions of the plan. A rider for
25 additional or increased benefits may be medically underwritten
26 and may be added only to the standard benefit plan. The
27 increased rate charged for the additional or increased benefit
28 must be rated in accordance with this section. For purposes of
29 this subparagraph, a person, his or her spouse, and his or her
30 dependent children shall constitute a single eligible employee
31 if that person and spouse are employed by the same small
