14         On page 1, line 29,

15         on page 2, line 14 and line 26, and

16         on page 3, line 22, after the period

18  insert:  A notice of merger shall be attached to the recorded

19  deed. Such notice shall include the name and address of the

20  current owner of the property, the name and address of the

21  surviving entity, and the date of the merger.
