15         On page 83, lines 1-5,

16  remove from the bill:  all of said lines

18  and insert in lieu thereof:  toll-free telephone numbers of

19  the Agency for Health Care Administration, the Statewide

20  Provider and Subscriber Assistance Program, and the Department

21  of Insurance. The consumer assistance notice must also clearly

22  state that the address and toll-free telephone number of the

23  organization's grievance department shall be provided upon

24  request. The agency is authorized to promulgate rules to

25  implement this section.
