11  Senator Holzendorf moved the following amendment:

13         Senate Amendment (with title amendment) 

14         On page 1, lines 14-28, delete those lines

15

16  and insert:

17         Section 1.  Industrial life insurance; application;

18  disclosure.--

19         (1)  Each policy of industrial life insurance in effect

20  before October 1, 2000, is subject to all applicable

21  provisions of the Florida Insurance Code and rules adopted

22  under the code.

23         (2)  Upon receipt of a written request no later than

24  January 1, 2001, each insurer that currently collects premiums

25  in this state for a policy of industrial life insurance must

26  disclose, on or before March 2001, to the policyholder or

27  premium payor the total amount of premiums paid, the cash

28  value, and the amount of the death benefits payable under the

29  policy. The insurer shall deliver a form to the premium payor

30  or policyholder to request the disclosure on or before

31  November 1, 2000.
