11  Senator Myers moved the following amendment:

13         Senate Amendment (with title amendment) 

14         On page 1, between lines 25 and 26,

15

16  insert:

17         Section 2.  Subsection (9) is added to section 465.025,

18  Florida Statutes, to read:

19         465.025  Substitution of drugs.--

20         (9)  Every pharmacy manager or owner representative of

21  a pharmacy shall report to the Board of Pharmacy any

22  information that suggests that there is a probability that a

23  prescription drug or device has caused or contributed to the

24  death of a patient or customer. Such reports must be made to

25  the Board of Pharmacy quarterly, beginning October 1, 2000.

26  The Board of Pharmacy shall summarize and categorize the

27  information by prescription drug or device annually and

28  present a report to the Legislature by December 1 of each

29  year.
