13         Senate Amendment 
14         On page 46, between lines 27 and 28,
15  
16  insert:  
17         (14)  Maintain in the medical record for each resident
18  a daily chart of certified nursing assistant services provided
19  to the resident. This record must be completed
20  contemporaneously with the delivery of care, by the certified
21  nursing assistant caring for the resident. This record must
22  indicate assistance with activities of daily living,
23  assistance with eating, and assistance with drinking, and must
24  record each offering of nutrition and hydration for those
25  residents whose plan of care or assessment indicates a risk
26  for malnutrition or dehydration.
