14 On page 48, line 25,

16 insert: (21) Maintain in the medical record for each
17 resident a daily chart of certified nursing assistant services
18 provided to the resident. This record must be completed with
19 the delivery of care, by the certified nursing assistant
20 caring for the resident. This record must indicate assistance
21 with activities of daily living, assistance with eating, and
22 assistance with drinking, and must record each offering of
23 nutrition and hydration for those residents whose plan of care
24 or assessment indicates a risk for malnutrition or
25 dehydration.
