17 and insert:
18 (4)(a) The biennial license fee required of a facility
19 is $300 $240 per license, with an additional fee of $50 $30
20 per resident based on the total licensed resident capacity of
21 the facility, except that no additional fee will be assessed
22 for beds designated for recipients of optional state
23 supplementation payments provided for in s. 409.212. The total
24 fee may not exceed $10,000, no part of which shall be returned
25 to the facility. The agency shall adjust the per bed license
26 fee and the total licensure fee annually by not more than the
27 change in the consumer price index based on the 12 months
28 immediately preceding the increase.
29 (b) In addition to the total fee assessed under
30 paragraph (a), the agency shall require facilities that are
31 licensed to provide extended congregate care services under

1 this part to pay an additional fee per licensed facility. The
2 amount of the biennial fee shall be $400 per license, with an
3 additional fee of $10 per resident based on the total licensed
4 resident capacity of the facility. No part of this fee which
5 shall be returned to the facility. The agency may adjust the
6 per-bed license fee and the annual license fee once each year
7 by not more than the average rate of inflation for the 12
8 months immediately preceding the increase.
9 (c) In addition to the total fee assessed under
10 paragraph (a), the agency shall require facilities that are
11 licensed to provide limited nursing services under this part
12 to pay an additional fee per licensed facility. The amount of
13 the biennial fee shall be $250 $200 per license, with an
14 additional fee of $10 per resident based on the total licensed
15 resident capacity of the facility. The total biennial fee may
16 not exceed $2,000, No part of this fee which shall be returned
17 to the facility. The agency may adjust the per-bed license
18 fee and the $200 biennial license fee and the maximum total
19 license fee once each year by not more than the average rate
20 of inflation for the 12 months immediately preceding the
21 increase.
