14         On page 8, line 30 through
15            page 9, line 10, delete those lines

17  and insert:  
18         (b)  The requirement of this subsection is met for
19  health insurance coverage policy forms offered by an issuer in
20  the individual market if the issuer offers the basic and
21  standard health benefit plans as established pursuant to s.
22  627.6699(12) or policy forms for individual health insurance
23  coverage with the largest, and next to largest, premium volume
24  of all such policy forms offered by the issuer in this state
25  or applicable marketing or service area, as prescribed in
26  rules adopted by the department, in the individual market in
27  the period involved. To the greatest extent possible, such
28  rules must be consistent with regulations adopted by the
29  United States Department of Health and Human Services.
