11  The Council for Smarter Government offered the following:

13         Amendment (with title amendment) 
14         On page 17, lines 26-28,
15  remove from the bill:  all of said lines
16  
17  and insert in lieu thereof:  include the applicant's social
18  security number. Notwithstanding any other provision of law,
19  the department shall be responsible for the printed or
20  electronic content of all initial licensure and licensure
21  renewal documents.  Such documents shall require information
22  including, as appropriate:  demographics, education, work
23  history, personal background, criminal history, finances,
24  business information, complaints, inspections, investigations,
25  discipline, bonding, signature notarization, photographs,
26  performance periods, reciprocity, local government approvals,
27  supporting documentation, periodic reporting requirements,
28  fingerprint requirements, continuing education requirements,
29  and ongoing education monitoring.  The application shall be
