14 On page 2, lines 14-23, and on
15 page 4, lines 4-13, delete those lines

17 and insert:
18 (a) Owned by, operated by, or having a contractual
19 agreement with the Department of Health and complying with
20 Medicaid's county health department clinic services program
21 specifications as a county health department clinic services
22 provider.
23 (b) Owned by, operated by, or having a contractual
24 arrangement with a federally qualified health center and
25 complying with Medicaid's federally qualified health center
26 specifications as a federally qualified health center
27 provider.
28 (c) Rendering dental services to Medicaid recipients,
29 21 years of age and older, at nursing facilities.
30 (d) Owned by, operated by, or having a contractual
31 agreement with a state-approved dental educational
