15         On page 2, lines 5-7, delete lines

17  and insert:  provide medical consent for a minor executed
18  after July 1, 2001, includes the power to consent to medically
19  necessary surgical and general anesthesia services for the
20  minor unless such services are excluded by the individual
21  executing the power of attorney.
