17 and insert in lieu thereof: evaluation can only be
18 administered by a practitioner licensed under the provisions
19 of chapter 458, chapter 459, chapter 460, or s. 464.012, and
20 in good standing with the practitioner's regulatory board. The
21 bylaws shall establish requirements for eliciting a student's
22 medical history and performing the medical evaluation required
23 under this subsection, which shall include minimum standards
24 for the physical capabilities necessary for participation in
25 interscholastic athletic competition as contained in a uniform
26 preparticipation physical evaluation form. The evaluation form
27 shall provide place for the signature of the practitioner
28 performing the evaluation with an attestation that each
29 examination procedure listed on the form was performed by the
30 practitioner or by someone under the direct supervision of the
31 practitioner. The form shall also contain a place for the

1 practitioner to indicate if a referral to another practitioner
2 was made in lieu of completion of a certain examination
3 procedure. The form shall provide a place for the practitioner
4 to whom the student was referred to complete the remaining
5 sections and attest to that portion of the examination.
6 Practitioners administering medical evaluations pursuant to
7 this section must know the minimum standards established by
8 the organization and certify that the student meets the
9 standards. If the practitioner determines that there are any
10 abnormal findings in the cardiovascular system, the student
11 may not participate unless a subsequent EKG or other
12 cardiovascular assessment indicates that the abnormality will
13 not place the student at risk during such participation.
14 Results of such medical evaluation must be provided to the
15 school. No student shall be eligible to participate in any
16 interscholastic athletic competition or engage in any
17 practice, tryout, workout, or other physical activity
18 associated with the student's candidacy for an interscholastic
19 athletic team until the results of the medical evaluation
20 verifying that the student has satisfactorily passed the
21 evaluation have been received and approved by the school.
22 (4) Notwithstanding the provisions of subsection (3),
23 a student may participate in interscholastic athletic
24 competition or be a candidate for an interscholastic athletic
25 team if the parent or guardian of the student objects in
26 writing to the student undergoing a medical evaluation because
27 such evaluation is contrary to his or her religious tenets or
28 practices, provided that no person or entity shall be held
29 liable for
