19 DISPUTES.--
20 (a) Every employer shall, if a request is made by an
21 insurer providing personal injury protection benefits under
22 ss. 627.730-627.7405 against whom a claim has been made,
23 furnish forthwith, in a form approved by the department, a
24 sworn statement of the earnings, since the time of the bodily
25 injury and for a period not to exceed 30 days prior to for a
26 reasonable period before the injury, of the person upon whose
27 injury the claim is based.
