13         Senate Amendment (with title amendment) 

14         On page 16, between lines 20 and 21,

16  insert:

17         Section 11.  Effective October 1, 2002, all credit

18  insurance enrollment forms must be approved by the Department

19  of Insurance pursuant to the provisions of sections 627.410 or

20  627.682, Florida Statutes.
