11  The Council for Healthy Communities offered the following:

14         On page 2, line 13, of the bill

15

16  insert:  When the copayment is payable to the insurer and

17  exceeds $300 the insurer shall allow the insured up to ninety

18  days from the date of the procedure to pay the required

19  copayment.  The insurer shall print in 10 point type on the

20  Declaration of Benefits page notification that the insured

21  could be terminated for failure to make any required copayment

22  to the insurer.
