18         (14)  The amendments to this section by this act apply

19  only to a major medical expense health insurance policy as

20  defined in s. 627.643(2)(e) which is offered by a group or an

21  individual health insurer licensed under chapter 624,

22  including a preferred provider policy under s. 627.6471 and an

23  exclusive provider organization under s. 627.6472.
