13         Senate Amendment 

14         On page 22, between lines 5 and 6,

16  insert:

17         (c)  For noninstitutional providers, must be mailed or

18  electronically transferred to an organization within 9 months

19  after completion of the service and the provider is furnished

20  with the correct name and address of the patient's health

21  maintenance organization. For institutional providers, unless

22  otherwise agreed to through contract, all claims must be

23  mailed or electronically transferred to an organization within

24  9 months after completion of the service and the provider is

25  furnished with the correct name and address of the patient's

26  health maintenance organization.
